Making an appointment is easy! Just follow the steps below:

1. Email us or Call Us with your preferred appointment time. Please indicate if you prefer to visit us in Greeley or in Lafayette. Angela, our clinic business specialist, will take your call, or will contact you via phone or email soon. You may want to list your phone number(s) so that we can reach you by phone.
   
2. After you have communicated with Angela, and after your  appointment is scheduled, Fill out your forms* 
   
3. Read the Patient Handbooks and complete the  "True/False Quiz"

*Before completing all of the forms, make sure you are eligible for a consultation (Step 1 above). All forms need to be completed before your appointment.
